Your Initial Evaluation

Whether you are self-referred to our office or are coming based on the recommendation of your primary care provider, urologist, or gynecologist, it is important that you bring copies of your medical records pertaining to your previous evaluation and treatments. These old records are vital to the management of your problem. You should call your doctor’s office prior to your appointment and ask to have your records be sent to us. If you are referred by a doctor, your records many have already been sent, but you should call to confirm. Sometimes it is better to have a copy of these records for your personal medical file and hand carry a copy to us. Before your initial evaluation you will be asked to complete a questionnaire. This is available electronically thorough our patient portal. Advanced completion our forms will allow us to better serve you, and evaluate and treat your problem. We will review your old records, your self-completed questionnaire, and will discuss these with you. Your examination will include a pelvic examination and urine examination. Depending on your individual problem, other tests may also be necessary. These may include cystoscopy, urodynamics, ultrasound, and X-ray studies.
