Scheduling Your Appointment
Your referring doctor’s office may have scheduled an appointment for you. You may also contact our office at 954-989-9998. We will need a very brief description of your problem, the address and phone number of your primary care provider or referring physician, and your insurance carrier.
We realize that our patients' time is valuable, and we make every effort to see patients at their scheduled time. We ask that patients provide as much notice as possible when canceling or running late for an appointment, With this advanced notice, the time can be scheduled for another patient.
What to Bring to Your Appointment
If you are a first-time patient, please plan to arrive at our offices early in order to complete a patient registration form. Please bring the following items to your appointment:
- Your insurance card
- Your driver’s license or identification card
- Physician referral forms if required by insurance plan
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
- Any recent test results
- Any old medical records that are relevant to your current problem
- In order to expedite your appointment and to allow Dr. Pollak to review your medical history, please download the following forms, fill them out completely and mail them to us at least one week prior to your appointment.
After Hours & Emergency Care
If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week.
Prescription Renewals
Prescriptions are renewed during normal office hours. In some instances, we may require that patient be seen in our office prior to medication renewal.
Financial
All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.
